Contact: Tina Urso, tlurso@seattleschools.org, Nurse, RN, BSN, NCSN 206-252-9407 (direct phone line); 206-743-3122 (secure fax)

Our school nurse, Tina Urso, is on campus at Gatewood alternating Wednesdays and every Thursday. She is responsible for acting as an on-site public health investigator under the direction of the Health Department, and is parents' main point of contact for any health-related issues pertaining to our students.

All students must complete a Health Registration form with current phone numbers and emergency contact numbers. The information contained will alert the nurse to any acute or ongoing health concerns that may impact students' abilities.

If your student is taking medication (prescription or over-the-counter) at school, the medication must be in its original container plus an Authorization for Medications to be Taken at School form signed by a doctor. This form needs to be renewed each year. If a student has a life-threatening medical condition it is required by law that the family provide emergency medication and a signed medication authorization form to the school before or on the first day of school.

Immunizations are required for continued attendance at school. You can read about the Immunization Requirements on the Seattle Public Schools Health Services General Information page.

Vision, Hearing, and Dental Screenings will be done within the first three months of school, and a letter will be sent home if it is recommended that your student get further testing. Contact Nurse Tina for additional information and a dental screening consent form.
